Welcome to Revive (“we,” “us,” “our”). These Terms & Conditions (“Terms”) govern your use of our website, clinic services, and facilities (collectively, the “Services”). By using our Services, you acknowledge that you have read, understood, and agree to these Terms and our Privacy Policy (including our HIPAA Notice of Privacy Practices). - You explicitly consent to receive appointment-related and promotional emails/SMS through the opt-in process.
- Messaging complies with CAN-SPAM, TCPA, and 10DLC A2P rules.
- Reply with “STOP” to opt out of promotional messages.
- You’ll still receive essential messages like confirmations or reminders.
5. Booking, Payment & Refund Policy
- A booking fee is required to reserve appointments. NON-REFUNDABLE. Applied to service total.
- Full payment is due at time of service. Packages may require prepayment.
- No refunds after service delivery. We may offer corrective options if needed.
6. Cancellations & No‑Shows
- Cancel 48+ hours before appointment to avoid fees.
- Cancelling within 48 hours may incur a fee up to 50% of service cost.
- Failure to show without notice (“No-Show”) may result in up to 100% charge.
7. Liability, Medical Disclaimer & Arbitration
- Services are elective aesthetics & wellness—NOT a substitute for medical care. Consult your healthcare provider for medical concerns.
- You release Revive and personnel from claims related to service outcomes, injuries, or property damage.
- All disputes are subject to binding arbitration in Charlotte, NC, under the Federal Arbitration Act. Small-claims court permitted if eligible.
- NOTE: Arbitration means that you and Revive waive rights to class actions and trial by jury for these disputes.
8. HIPAA-Compliant Health Record Access
- You have a right to inspect, amend, and receive your PHI.
- PHI can be requested via secure, HIPAA-compliant methods.
- We keep health records as required by law.
9. Photography & Media consent
- You may consent in writing to allow “before-and-after” photos/videos for marketing or education.
- You can withdraw consent for future use, but cannot retract media already published.
- We’ll obscure features to maintain anonymity if you request it.
10. Service Adjustments & Pricing
- Prices vary by service type, duration, material cost, and practitioner expertise.
- We reserve the right to adjust pricing where clinically justifiable.
11. Third‑Party Processors & Data Security
- We may share PHI with licensed third-party service providers—only as needed and under confidentiality.
- We use industry-standard security controls, but cannot guarantee complete protection. You consent to residual risk.
12. Cookies & Tracking
- We use cookies (functional, analytics, third-party) to improve experience.
- You may disable cookies in settings, but site functionality may be affected.
- Continued browsing indicates consent.
13. Children’s Privacy
- Our Services are not for children under 13.

- Liability Cap: We are not liable for indirect, special, incidental, or consequential damages—even if advised of the possibility.

We reserve the right to refuse service due to:
- Abusive or unsafe behavior
- Clinical contraindications or unrealistic expectations
- Medical unsuitability
We may offer referrals or alternate options in such cases.
19. Severability & Governing Law
If any provision is invalid, other Terms remain effective.
These Terms are governed by North Carolina law and any dispute must be brought in North Carolina courts or arbitration as set out above.
